Vermont License Renewal..HELP!

Hello guys! I need help. This is my second time to renew my license in Vermont, however due to lack of employment opportunity here in PI, I havent been into any work experience since getting my license last Dec 2006.Would the VT-BON renew my license?

Hi Toma, when did you receive the form from Vermont BON? My license will expire on March too but I haven't got forms from them yet. I wonder if they send all or if you request for the form.:icon_roll

i received it the other day. you dont have to request for it, the bon sends it to all the licensee. but in case that u dont receive urs. u can download the form in their website. its already available there.

i had it renewed.. the form doesnt say anything about having the license inactive..

hello! I just want to ask about the bank draft for the renewal fee. I was really confuse to whom to address or what to indicate in payable to:

In the renewal application form, it did not indicate the beneficiary's name.

I read VERMONT BOARD OF NURSING from the letterhead of the renewal app. form. So I decided to put VERMONT BOARD OF NURSING as the beneficiary's name on the bank draft/ check.

Now that I have the Bank draft I realized maybe the correct beneficiary's name is Vermont Secretary of state.

Do I need to ask the bank to change the beneficiary's name to VERMONT SEC OF STATE?

or will they accept the bank draft payable to : VERMONT BOARD OF NURSING w/ 95 usd?

Please enlighten my thoughts...

Beneficiary's name should be VERMONT SECRETARY OF STATE. I doubt if they will accept VT BOARD OF NURSING. You can request the bank to change it though.

hi! mazern. thank you for the very early reply.

i went to the bank and i told them to change the beneficiary to vermont secretary of state.

they just erased the previous input then they signed near the erasures or corrected part..

i can send the check together with the application forms on monday.

you really made my worries gone... :-)

hi!

On the last page of renewal form it says on question number 1 :

1. I garduated from my entry level RNeducational program within the last five years. Yes___ No____

If YES date of grad _________

If NO complete questions 2-5below

and the RN employment is found on number 4 question.

I think we dont need to fill up the RN employement.

kindly correct me if I'm wrong

I have a question, what would you place in the name of nursing program? I am from the Philippines. Is it safe to just state Bachelor of Science in Nursing or is there an other answer? thanks :D

i wrote bachelor of science in nursing.. but im not sure if its correct.. what did u tick on the highest level of nursing education u have completed? did u choose diploma?
