Transcript Validation Form

i received a letter from cgfns saying they still need transcript validation form. can someone tell me which form is this exactly as i remember sending all the forms from the booklet to my college. i was very careful, but maybe missed it.

what service are you requesting from the cgfns? is that for ny-cvs, visa screen certificate (vsc) or cgfns certification program (cp)?

if it is for vsc, ces or cp i think transcript validation form is the 'nursing education form' to be filled and sent directly by your school.

if u had applied for ny-cvs the transcript release authorization (signed and attested) has to be sent.

hi, starbin, thank u for ur reply! i've sent all the documents required for vsc by cgfns. if it is the nursing education form, isnt it provided with all other documents in the booklet.

do u know how does it look like. is it with like the transcripts with the hours in it?

elenova, you have to submit and sign the Transcript Validation Form then give it to your school so that they could send your Transcripts to CGFNS ICHP....

Strange, my College has already send them the transcripts. Do u know where I can download the same form from?

It's in your VSC application.

But if you haven't signed it, your school has to do it all over again.

The top part of that paper you fill out, and the rest is for your school. Then your school needs to send it to CGFNS. And make sure they seal it.

Otherwise CGFNS won't accept it.

This is exactly what I did, I filled the upper part of the paper and mailed it to the College. Maybe I have to do it again. I better called them to check directly with them.

elenova, the way i see it, your school received the transcript validation form BUT they did NOT mail it to CGFNS ICHP together with your transcript....only your transcripts were received by CGFNS ICHP :uhoh21:

Actually, my mom mailed the whole package with all the documents as my College will not pay for the postage price. The College will only fill the papers, they will go to the translator, come back to the College, then again to the translator and the last, signed by the director in the College, the envelope is stamped and closed and given to my mom, only then she is sending it by courier to make sure it is there. It happen many times that my documents were lost on the way. Lets hope we will do it right this time.

I believe you can download it in http://www.cgfns.org on the VisaScreen application packet.

i thought that cgfns will use the school records the nurse submitted when she applied for cgfns certificate, that they no longer ask another set of records because they already have them. i was wrong then

I looked at my documents and I have filled and sent to the College 2 documents: "Request for Validation of Registration/License for Visa Screen" and "Request for Academic Records for Visa Screen". In addition to this the official transcripts were sent. Which one of this is the "Transcript Validation Form"?

Any idea?

Your school needs to fill out the request for academic records.

The other form is for BON.
