I'd had this lady the day before and she was totally alert and oriented. The next day in the morning she was a little confused and kept thinking she was no longer at the hospital, but she was redirectable. We got urine sent which came back dirty. Got an IV antibiotic ordered. She also needed a blood transfusion, but I was waiting for the antibiotic to come up and give that before starting the blood. I went to lunch and the antibiotic came up. When I went to give her the antibiotic she was sitting on the side of the bed trying to stand up on the post operative knee and her IV was horizontal instead of vertical in her arm. She was yelling and trying to get out of there because we were going to take her blood and that we were all vampires and she could get the blood and that I the nurse couldn't get the blood. She also said that the doctor should get the blood, why are the nurses going to do it. She was so scared, poor lady. I got a couple people to get her down, tried to flush the IV to give the antibiotic, it was gone, and called security. She was yelling down the hall that we were all going to kill her. My other not totally oriented patient was getting concerned. What are they doing to her?, she asked. The patient next door had her door closed and was on the phone with her mom and her mom asked if that was the TV. Not only that, but the patient called her daughter all freaked out and then her daughter called ME equally freaked out and I had to calmly explain what was going on and that she could call back later tonight. 5 mg IM haldol later and I had a much calmer (yet now suicidal) patient...who agreed to go on CPM and barely flinched when I inserted a new IV and finally got that antibiotic into her at 7pm!

This is when I was an aide, don't work with adults currently. Elderly woman admitted with change in mental status, found to have UTI. Told a very convincing story about how she'd gone to a concert in Boston the night before. She took a cab home, she said, and when the cab dropped her off she accidentally wandered through a tunnel which lead her to the hospital and that's how she ended up here. It was a mistake, didn't we understand, so we needed to just let her go because she really didn't belong here...

The case of the patient in the hospital in SanFran who disappeared only to be found dead in a stairwell some time later. When that story broke initially I read that she had AMS from a UTI (later on it also came out that she was a severe alcoholic,I'm sure that had as much to do with it as the UTI).
