I was having this discussion with a friend, but we later agree to to these. Hope this will benefit all nurses student still in school or awaiting to take NCLEX. Is more or less like a study :rckn:group. If you think you have any question you can thread and pple will respond with answer. a pediatric pt is admitted to the hospital with a diagnosis of bacterial menigitis. what type of nursing care should be included

1- provide a well lighted environment

2- maintain isolation for 72hrs

A nurse read the physician chart with an order for prednisolone 15mg PO for a pt with cirrohosis but the medication in the pt medication box is 5mg.The nurse should take which action

1- call the pharmacy for clarification

2- call the physician for clarification

3- notify the next shift for clarification

4- give the 3 tablet of 5mg PO

place in other of priority for a pt experiencing chills,headache, flushed skin and temp of 101*F during blood

transfusion

1- obtain blood culture from the pt

2- infuse NS to keep the vein open

3- send the blood bag to the blood bank and administer another set of blood

4- stop the blood infusion

1- insert foley catheter

2- inspect groin area for wetness

3- apply perineal pad and change frequently

4- turn pt every hr

5- wear incontinence briefs

A nurse should intervene if she notice a CNA preparing a bed for a pt who have knee surgery 2days ago

3- side rail @ the hob and foot of the bed are in up position:up: side rail are considered restraint if both side are used. if the side rail @ the hob are used, it will help the pt in sitting up and are considered safe.
