SJSU Spring 2017 (Questions/Comments/Concerns)

They really should have mentioned something about getting some of the stuff done after Dec 10 because I bought my Liability Insurance already and it expires 11/29/2017 so now I'm gonna try calling them to get the date changed.
For the CPR certification, I picked the Basic Life Support for Healthcare Providers from American Red Cross and I'm taking the class on Dec 10

I am on the same boat as you!! I just sent NSO an email explaining the situation and will call tomorrow during business hours to speak to someone.
I can't do the e-sign for the background check so I have to print it out and email it to them, but what did you all check for under the "Notice regarding credit checks" Did you just check them all?
I printed mine out and emailed it to them and I didn't check anything under the check list.
Don't worry, I was confused about everything too! I had asked her about the Immunizations record being all on one sheet and Dr. Rauch said that you can upload that for each Vaccine that is listed on CastleBranch. I have my immunizations record and was just going to do that but she told me it would be best to get the titers so that way they know we are for sure immune, but you can upload your immunizations record if you want, they just prefer the titers lab report.

2 important things:
1. If you have immunizations already it is still preferable that you get the titers so that you know for a fact you'll be safe if you come into contact with infected patients!
2. You need to get them AFTER Dec. 10th and before the deadlines so that they don't expire during school time. In other words, while you are in contact with patients.

Yea they could have been clearer, but for the Liability Insurance you had the option to say when you wanted it to be active. I requested it to be active on the 23rd of January 2017 so itll expire on that day before classes start.

I was able to do it online, but if you're not sure download it and it should filled up correctly for you. When I did it, the form was filled out for me so I just had to click the e-sign button and I was done. Try doing it like that just to check what was checked on the form, if that's the problem.
Also for the documentation of influenza, do we request documentation/file from the clinic or do we use this form http://www.sjsu.edu/nursing/docs/flu_documentation_proofrev6.19.14pdf.pdf from the School of Nursing website to have the doctor of the clinic fill out & sign? I got my flu shot at a clinic.
