SJSU Spring 2017 (Questions/Comments/Concerns)

Yes you do a quote and then you sign up for it its an automatic process at least it was for me

For profession I chose Registered Nurse since thats what we're preparing for, I wasn't really sure I should have emailed to make sure

I didn't see registered nurse as one of the options so I was picking something else which was confusing me even more but I got it now. Thanks :)

For all the additional documents that we need to submit like our vaccinations, are you just getting documentation from your doctors that says when you got your vaccinations and when? I'm becoming overwhelmed with all of the additional stuff we need to do, especially since I'll be out of the country once school ends until it starts again and have to get everything done before then

The background check and drug test are needed first so take care of that.

I have Kaiser and can access some information online. For everything I need like health statement and vaccines, one Dr visit should take care of everything.

Don't worry you'll get it all done. Just make a list and tackle it.

Hi guys, so just double checking...all we needed to do for the background check was to fill in the blanks with our name, address, driver's info, county, and sign/e-sign the disclosure form, and submit right??? (i feel like i didn't do it right) Also (I think some of you have already mentioned this, but) do we need to wait for the email from SupportFormFox before we do the drug screen or can we just go ahead and make an appointment for it??

Sounds like you did it right. Should show up as "complete" very soon.

For the drug screen, I downloaded the form and with it came a list of locations for Quest Diagnostics. I made an appointment.

I had my appointment today. Just took the print out with the barcode and I was all set.

I talked To Dr. Raunch and she said its better to just get Titers for HepB MMR and Varicella. She mentioned thats its more for our safety. Basically, make sure youre immune to these things, you could just upload your Immunization record that shows that you have them, but its best to get checked and make sure we are protected 100. (I may have used some terms incorrectly)

Also, she said we are supposed to get these things done AFTER the 10th of Dec. and before the deadline. Titers/vaccinations TB testing

except the flu shot, as log as you had it in this current season you should be good for that one.

Does anybody have any idea where we can do these titer tests? I was thinking that maybe SJSU's Health Center has these services but I don't know...
