I work in a Internal medicine clinic. I got an order to give 250 mg of Rocephin IM. Mixed with 0.9 ml of 1% lido the dose equaled 1 ml. I gave this to the pt in his deltoid as I was taught you can give up to 2mls in the deltoid. This was a otherwise healthy young man (besides his current situation) and he had a well developed deltoid. I know IM Rocephin hurts but I gave it slowly and he said it wasn't bad. I would like your honest thoughts on wether this site was Ok or if I should always use the glut with Rocephin.

Ventrogluteal is the site used in our clinic. We generally avoid giving antibiotics in the deltoid. What does the current literature say?

I bet he was sore the next day, as you know Rocephin is very irritating to muscle so in the future I would definitely only give it in the Ventrogluteal as the other poster said.

As far as I know, the package insert just says it should be injected "well within the body of a relatively large muscle."
I personally don't use the deltoid for anything much other than immunizations. The VG or VL are my preferred sites.

In a healthy adult male, it's probably fine. Nevertheless, for Rocephin I prefer to give it VG unless they absolutely insist on deltoid.
Ok then...it's seems I will use the VG site from now on for Rocephin. Although this young man was built and had a very large well developed deltoid
