Resignation and thoughts?

I've been feeling pretty burnt out in primary care over the past 8 years; have changed jobs 3 times and about to make a change for the 4th time, although this time into an inpatient specialty position. Can I get some feedback on wording and what to say/how to approach my supervising MDs? Having extreme anxiety! It's the first time I'm leaving a job where I do enjoy the people and staff that I work with - even the management. It's just the nature of primary care that I'm tired of. 

Does this sound appropriate? Do I say less? Or more...?

Is it too much to talk about where I'm going/what hospital I'm going to? 

I work inpatient and can tell you that this setting will burn you out in the same way. Gotta find other ways to manage burn out other than switching jobs.

Don't bother staying on as per diem, just cut the cord.

Yes. All you need is please accept my resignation effective xxxx. Check your contract if you have one to ensure you give adequate notice to get any PTO etc paid out.
