Do you legally have to tell a patient if you accedentally give them an overdose of a drug and the patient has a seizure as a result of the overdose?

I'm not sure about the legalities. I was told by an instructor to never report errors to the patient, as this immediately incriminates everyone involved.
With that said: I think patient should have a right to know if an error was made and steps done to correct it. We need to be accountable.
