Vanderbilt was culpable and contributed to this sentinel event. That is the Swiss cheese model. This could have been prevented,  obviously by the nurse, but prevented by the hospital as well.

There were a lot of inaccuracies and omissions in that article which is not at all surprising. But please tell me Vanderbilt is culpable when a nurse doesn't even look at the vial of medication she is about to use?

Her role should have ended with her sentencing - she has nothing of value to offer in nursing or healthcare, but something about her hitting the rubber chicken circuit plus the folks willing to exploit her as the conference equivalent of clickbait makes it all sordid and disgusting

Neither of those options remotely come close to $10,000

As for having compassion, she bypassed multiple checks, including not reading the dam phial. As a result Charlene Murphy died an excrutiating horrible death, suffocating to death and fully aware that she was dyinig. Unable to tell anyone

Which sucks on two levels. RV slammed the medication and left her alone. Charlene Murphy was comopletely paralysed and unable to alert anyone

No it wasnt just a mistake, she didn't read the phial of medication. VERCURONIUM vs VERSED, Never mind the 8 plus other safety checks she bypassed. The whole tragedy could have been averted if she had just read the bottle. 

She has done nothing to accept her mistake. It was someone else that found the mistake. We've heard all about how vandebilt was at fault. Not once has she ever said "I screwed up"

Genuinely remorseful people don't try to financially profit of their wrong doing. I doubt your friend is about to jump on the speaking circuit charging such obscene amounts of money. At one point she was also doing cruises and again charging obscene amounts of money for the privlidge

No, it couldnt. 

I had to draw up vancomycin the other day. Do you know the first thing I did? Checked the box to ensure the correct medication, checked the phial in the box to ensure I had vancomycin. I then checked the EMAR to ensure I had the correct medication for the patient

Charlene Murphy would have been alive had RV read the label. 

VERCURONIUM, VERSED apart from the first intital the two drugs arent remotely alike.

How is a hospital responsible for a nurse not reading the medication label?

don't get me wrong, not to say the hospital has massive systemic errors at the end of the day she didn't read the label and Charlene Murphy died a horrific death
