Writing “poop” is unprofessional?
We had a GI doctor that chastized our baby nurses for just putting the Patient had a BM. I wrote: Patient had a copious amount of brown, bright red, black semi-soft to solid-hard (rock-like) feces. The Patient strained to the point of vasovagal reaction to produce BM. Could not flush down the toilet maintenance called to clear the blockage. Provider notified, will write new orders.

War on Nurses
I am Faculty at a private-for-profit college; I teach in the LVN program, either 12 months full-time or 15 months part-time nights. Both programs are fast-paced. Students are actively engaged in Theory/ Clinical Learning Lab, Simulation Lab, and Clinical sites. We encourage our students to stay actively involved in all aspects of their education, and one focus is critical thinking versus task performance. The college also has an LVN-ADN program, BSN and MSN programs. The college is rapidly expanding to different states/locations. Due to a faculty shortage, we have students waiting to attend; the pay isn't competitive with PRN positions in the industry, so many candidates walk away. So how about improving our income also. Another aspect would be recruiting from the high schools; many districts already have agreements with local colleges for students to attend college-level courses and receive credit. Make it mandatory for colleges to accept those courses if they meet the state accreditation standards.

IVPB simultaneous or not?
What does your textbook say? Do they want you to demonstrate manual versus pump? We use manual setups when we do not have IV pumps available. In addition, you should be able to calculate the drip rates correctly; pumps can malfunction also. Where are you getting the list of secondary lines that are must not share?

Predatory New Grad Contracts
WIth HCA contract, you can complete the contract at any HCA facility, so perhaps just transferring to a different unit/facility would have worked for you. I have seen that multiple times at the facility I worked at.

Why did she not have her own keys? Were you drug tested after the incident? (CYA)! Write down the events of the day while they are still fresh in your mind. State only the facts. Keep this for your records sometimes these investigations can take a while.

Help. My clinical group dislikes me.
What are you in high school? This in incivility at it's best and will not be the last time you encounter it I am sure. Is there an age difference in this group? Can you be assigned to another group that would be better suited for you? Nursing school is hard enough without all of that crap going on. You be you and concentrate on your end results, becoming a safe, compassionate, self-sufficient nurse. You got this keep going, leave them behind. Find other classmates outside of this group to work with.

HELP! Need to make decision!
Phlebotomist vs CNA stress level would be less as a phlebotomist. I believe they also make more money? I agree working as a CNA can open doors for future nursing positions and help develop skills. Make sure that if you are performing tasks while working as a CNA that are RN's tasks that you have permission from your facility (in writing) as this is a liability issue should something go wrong.

Need Advice on Accused Medication Error
Wow, what a shame. I am wondering how this situation got so blown out of proportion? Was the instructor at the review process? What was the story relayed to the other panel members? And were there any of your peers on the panel for review of the situation? Has there been tension with this instructor? Were staff at the clinical site involved, i.e. was it discussed with anyone on staff, if so in what context "medication error"? When I was in ADN school we had two incidents with an instructor who targeted students. She literally got one student removed from the facility and expelled from the program. She literally had two other students in tears and afraid to come to clinical. Since you are in your first semester do you have options for another school?

On call policies
Call on our floor is based on staffing only. The order of who to put on call starts with PRN employees, then Core, people in overtime are asked. They will consider people who volunteer also. They keep a log of who is placed on call, floats, code team to try and keep things fair.

At HCA any gift they give you is valued and taxed. So if they give you a $50.00 Coffee card they report it as income and take taxes off for it.

Another nurse fired from HCA for speaking out against staffing
Staffing issues are horrible with HCA, they staff by the number of patients, not acuity. One time I was split between the East and West side had a patient on each side towards the farthest wall one at the nurses station, three with CBI's running (2 were fresh post-ops) and a fresh bariatric r-y surgery, and one psychiatric patient who had an unstageable pressure ulcer big enough to place your whole hand in who screamed the whole shift and ripped his dressing and packing out 17 times that shift. A typical day at HCA and a horrible charge nurse.

Controlled med not documented
Been there done that, use it as a learning experience. Make sure in any write up that you state what you are going to do to keep it from happening again. Such as I have changed my practice after scanning the patient, I scan the medication and I submit the medication within the system, I then administer the medication. I then double-check to make sure the time of administration is there and record the time so that I know when the next dose is due.

Safely working with patients with autism
There are people on the spectrum who also have mood disorders, such as explosive mood disorder. Also, the more people involved in the situation, the more reward they are getting as at times it is about getting a reaction/attention from someone.

Facility does not provide breaks - What can we do?
I live in Texas, and the organization that I work for is in the network of the hospital that the nurses sued. We are now mandated to take a 30 minute lunch period. However, they do not provide additional staffing to cover the period. So your "buddy nurse" has to cover your patients, which puts you at ten patients for 30 minutes. The latest they are saying is that when nurses do not take their break, it causes fractional overtime and that they will now start writing nurses up who do not take a 30-minute break, up to termination of employment. Meanwhile, they are pressuring you to get patient's discharged, most times no CNA support as one CNA has 10-20 patients, acuity levels not taken into consideration, total care patients and patients who require two people assist to ambulate. As soon as a bed is empty, it is being filled. I have discharged three patients all within 30 minutes of each other, and it takes housekeeping 10 minutes to stat clean a room, so my first patient arrives while I am discharging the second patient and the second one while I am discharging the 3rd patient. Oh, and patient number four needs a brief change and patient number five need to get to the commode. So on days like this when they want me to take an uninterrupted lunch is beyond me. Oh, and we are required to wear our "walkie" and respond while on lunch. Most nurses on the unit fear reprisal, so they clock out and don't take a meal break. The problem I see with that practice is that if you are injured on the job or make a mistake can you be assured the company will cover you. Probably not.

Where are you applying? Are you being to selective? Have you tried home health, SNF. Have you opened yourself to all shifts?

We only get a 30 minute lunch break and are required to keep our headset on. When someone goes on break we cover for each other so yes we will have 10 patients no one ever says anything about it. But don't take a lunch and get fractional overtime and they go berserk.

Is it possible for a nurse to become a doctor?
You can do it if you really want to, I know several nurses who went on to become doctor's. I know a few doctor's who have wished they became nurses instead too. There will be a lot of debt for med school so are you prepared for it?
