I also did horrible on the NCSBN questions. I did the 3 week course as well and don't think I ever made a passing score! I took the NCLEX in 75 questions and passed despite those darn NCSBN scores, lol!

I checked the PVT about an hour after my test, and got the good pop up. I proceeded to check it another 6x in the past two days until I got my quick results now, showing a pass!

I think that requires two conversions. The first would be the ml per dose from the formula of dose/have X mL= how many ml to give. Then I guess you would do that mL/time in minutes (45) to get your answer. I highly doubt you'll get anything like that...
