pre-program physical documentation

I had a full physical in April. I needed a report to submit for my nursing program. My doctor gave me this 6 page printout that includes everything. I have to submit this through a third party company as part of my background check, along with all my immunization records, health insurance etc.

I am very uncomfortable giving out this massive chunk of information. Can I ask her for a form letter to submit in it's place? What would you do in this situation?

My college had a letter that the provider signed off on. It included a couple pages of exactly what the college was looking for (sound mental health, able to stand for long periods, etc) but it did not include any Personal Health Information. I would definitely ask if there is something else you could submit.

There should be a 1-2 page document you can use. Ask you instructor. The only thing the school wants to know is if your are physically capable of attending clinical. For my school, I had to have a physician, NP, or PA sign off that I do not have any conditions that prevent me from safely completing clinical and then they had a short summary of the physical requirements of the program and had to sign that I was physically able to participate. That was it because the rest of your health info is private to you. You dont give up your HIPPA rights just because you are going to nursing school. GL and CONGRATS!
