Hi All!
Im writing my first care plan and I have come accross a little hitch. I am writing interventions regarding a developmentally delayed child but the interventions are aimed at easing the mothers worry as the child is growing well. Only has problems with communication (not severe) related to lack of social interaction. Now when I write the report should I identify the mother as the patient or the child? The child is 4 and I have reffered to speech pathologists etc etc etc, but as the mother is receiving the educational benefits from these specialists does this make her the patient?? For example referal to a parenting program to help her teach her child to use more language when communicating.
ARGH, I may just be making this more complicated than I need to..... IM LOST..:)

The child would be the patient.The mother would be the mother.

actually, they are both your patient. you were right the first time-- address the child's status and the mother's issues and needs regarding her child's status.
don't believe me? look it up in the ana scope and standards of practice.
