It either seems to me you totally don't understand nursing dx, or haven't put much effort into your assisgnment. What have you done so far?

Either way, refer to the link below for help with careplans.

I don't mean to sound harsh, but you need to learn how to write care plans. You WILL use them throughout your nsg career:

well that was harsh. I am in my senior year and have done my fair share of careplans. Yes I know how to right them and when I look in my nursing diagnosis book there are only 3 options for hypertension and they have to do with noncompliance and that isnt the issue for my client. or imbalanced nutrition which I could use but my patient didnt speak therefore I couldnt assess her knowledge of the disease.

so far I have looked up all 47 labs that are normal and abnormal and explained why the abnormals are that way. Looked up all my medications and wrote action, use, nursing considerations, wrote a paper on the patho,

Ok, I am a student and will try to help. I bought a used text by Ackley/ Ladwig that helps translate Medical diagnosis into possible ND. I looked at hypertension: #1- Imbalanced nutrition: more than body requirements r/t lack of knowledge of relationship between diet and disease process- #2 Ineffective health maintenance r/t deficient knowledge regarding treatment and control of disease process. # 3 non-compliance...

Hyperglycemia: Ineffective management of therapeutic regimen r/t ...many choices (def knowledge, economic difficulty, etc.)- #2 Imbalanced nutrition more than body requirements r/t intake of excess intake of nutrients.

Hope this helps

You can't completely rely on the book.

Every patient is unique.

If it's not noncompliance, then what are the other issues going on r/t hypertension?

If you are struggling to come up with four and cannot complete one...

I'm not sure you do get it and I'm not trying to be mean either.

or imbalanced nutrition which i could use but my patient didnt speak therefore i couldnt assess her knowledge of the disease.

that statement, right there, brings up another nd.

Decreased cardiac output causes hypotension not hypertension. I think this is why people are questioning how much research you have done.
