EMS call, glucose of 7. Couldn't give him gel b/c he was unconc (you think?!), before the days of glucagon or starting an IV in the field. Got him to ER, IV established. Awake, talking, wanting to leave all before we got our paperwork finished!

This is why I was always grateful for the instructions of an endocrinologist back in the 70's to keep sugar packets nearby. He explained that sprinkling one under the tongue would instantly raise hypoglycemia with no threat of choking. Used it many times in my career.

All same pt:

lactic acid: >16

pH: 6.35

bicarb: 3

rectal temp: 33

We have those patients ALL the time. Means they'll never be able to wean them, so let's let them struggle not intubated and see what happens.

RT said he wouldn't even survive the actual intubation/the change in CO2 needed to establish an artificial airway.

Saw a HGB of 4, with K+ of 8, with rhythmic changes in ICU yesterday.

Pt was acutely rejecting transplanted kidney.

Good grief was there any elasticity left in the person's heart?! I'm picturing the 2D Echo showing an EF of 2%. Holy crap! I didn't know a BNP could go that high!

His BUN was >100. He was a dialysis patient but fistula wasn't mature yet.
