Received my DL today!!! Sort of...well it's not mine, it's my wife's. At least it felt like it's mine. My wife and I sent our documents together but her application was queued earlier. She was placed on queue on the 12th of August while I was placed on the 18th. She got her DL today Sept. 22nd thats exactly 40 calendar days of waiting! Exciting!!!

The form to be filled out by PRC and those you mentioned! And of course the other documents to be uploaded on your NMC portal like your birth cert, NBI, college diploma, PRC board cert.

Oh yes it is. You have to attach it on the school document. Your school registrar or the school staff that fills out the form will have to attach an original copy of your TOR and RLE on that form.

You might want to include an original/notarized copy of your COE, in case nmc needs it in the assessment process. The health declaration doc is usually accomplished by the agency, try asking your agency's point of contact.
