The declaration of good health is one of the forms you download from NMC, thus the 9-month validity applies.

Go to your account, in nmc, and beside the download link, it will appear there that the forms are already recieved usually akes 10 to 15 days before it appears, you can check in dhl thru your trackin number if it has arrived in the destination. Usually 3 days only.

Its good to know that you already sent your files to nmc.
