Hi,
I'm new to this site so I did not know the best place to post this. I just started work with a small in-home services agency and I'm not able to contact the previous nurse. On the nurse visit report that we use there are 2 sections that I'm not familiar with. The first is LOC and the second is P/Risk. When I look through past visits the LOC will be documented with numbers, usually 24 or 27. The P/Risk will usually be 2 or 3. Can anyone help me with what thses are? I'm embarassed but I don't know who to ask because none of the other staff are familiar with the nursing terms.

LOC= 24 or 27? I've only seen person, place, time.
there should be some type of key w/ your pt risk.

YOu need to ask your co-workers.......just say you afre unfamiliar with those terms. There are certain requirements by medicare that need to prove a certain care level for reinbursement or they remove care. \Better be safe than sorry!
Thanks for your replies. The problem is that no one here knows because I'm the only nurse, at present and I can't reach the previous nurse.
I will keep trying to find out. I've only been familiar with person, place, time as well.
I'm happy to report that I found the answer on one of the state forms we use!

so, whats it mean??
It has to do with the person needing to meet a 21 point system to qualify for Medicaid In-Home services. So a person who gets LOC (Level of Care) 24 definitely qualifies. The P/Risk is the risk level of living independently (i.e. 1 is low, 2 medium and 3 is high risk).
