I just got a job at a SNF (Skilled Nursing Facility) that focuses on rehab. I start my shadowing next week.

I would love to hear from you all who have worked in a facility like that and what your 10 most common Nursing Diagnosis were. Thank you.

Well Rehab's a little different, but in my experience many SNF patients have:

Stroke

Dementia

Congestive Heart Failure

Coronary Artery Disease

Chronic Obstructive Pulmonary Disease

Emphysema

Asthma

Diabetes Mellitus Type II

Chronic Renal Failure

Urinary Tract Infection

I think these are the top 10, but everyone's opinion may vary a bit.

Here are some common diagnoses that are seen at rehab units in skilled nursing facilities:

Congestive heart failure

Type II Diabetes Mellitus

Status-post TKA (total knee arthroplasty)

Status-post THA (total hip arthroplasty)

Debility

Status-post CVA (cerebrovascular accident)

Status-post MI (myocardial infarction)

Urinary Tract Infection

Wound Infection

Status-post pneumonia

Risk for injury r/t whatever - They are ALL high fall risks

Altered elimination - they are all constipated from immobility and narcs

Impaired mobility - post-op, weak

Self care deficit - can't perform their ADLs without assistance

Acute pain

Chronic pain (old folks with lots of arthritis)

Risk for infection - surgical sites, pneumonia r/t immobility
