I would like to get further clarifications for those who are familiar in applying for registration in Australia through APHRA. There has been a new forms used with the AGOS-40. I am planning to do it all by myself since I'm on a tight budget. Can someone share their experiences and important tips in filling up the said form?? Thank you!! Your help is very much appreciated.

Sending an application to AHPRA for registration is fairly easy. We just need to pretty much read. Everything is stated in the AGOS 40 form including all the requirements which is written in every number. It's all straight forward requirements and simple. i.e Question 12 in AGOS 40. What is your health practitioner registration history. State the title, examining body, campus if any and the dates of issue and expiry. There's a box there that says you need to provide certificates of good standing if you have been registered outside Australia. And that is all the countries you were registered in. Another tip is to look for Section L in AGOS 40. All the documentations/requirements you might possibly need in every question is there.

Can someone share their experiences in completing the Agos 40 form?? Im very confused with some of the requirements needed?? Then, where do i need to check my criminal record??? 'Coz i've heard about international criminal history check?? Also, do i need to send my tor, diploma, rle directly to aphra, or my school will be the one who will send it to aphra??

You can send a certified true copy of your diploma and RLE directly to AHPRA.

Section C. Proof of identity

Category A - I dont have any proof from this categeroy

Category B - I have my PH passport but dont have Aust. Visa

Category C - Board Registration Cert. , is this the same as the registration certificate from PRC? Also, health insurance card, is PhilHealth consider?

Category D - None.

Section D

5. Should i eliminate "0" from my mobile number since thay are asking for 10 digits?

6. Is my residential address same as my home address or different??

Do i really need to ask PRC to send my registration certification directly to APHRA? Also, my pharmacology certification??

Like I said, all the answer to your questions is in the form itself if you READ it.

- Proof of identity. Section C. If you read this, then you would've seen the red letters. Please go through it again as you don't need to supply all the categories as an overseas applicant. In fact, you only need your passport and have it certified true copy as an identification. This should meet the minimum requirements needed as you are not in Australia yet. However, you can always add more documents such as your birth certificate, Int'l driving license etc. Click here to read about this. This link is also written in red letters.

Section D.

5. Change "0" to "+" - It'll mean "00" Or you can still use 00, they don't necessarily need to be fitted inside the boxes

6. Residential address is your home address.

7. Yes, if you put no, then you are to provide your Australian principal place of practice, which obviously, you don't have.

You need PRC to send Certificate of Good Standing directly to AHPRA and not registration certificate as you already have that in hand. (Board certificate).

You can obtain pharmacology from school and send it directly. This will not change anything as you will not meet that criteria anyway.

All these answers are based from my own experience and understanding. You are always free to ask someone else should you not be satisfied. Again, there is no better way than seeing it yourself, and that comes with reading and understanding. :)
