Dear Mr ....
Re: Application for General Registration - Registered Nurse
I refer to your application for registration received by the Australian Health Practitioner
Regulation Agency (AHPRA) on 18 March 2014. An initial review of your application has been undertaken and it has been identified that you have submitted an incomplete application. Your documentation provided does not constitute a complete application under section 77(2) of the Health Practitioner Regulation National Law (National Law) because you
did not provide all documentary evidence.
If you wish to proceed with your application you are required to provide the
information/documentation outlined below:
Statement of Service
A satisfactory statement of service is required from your employer/s over the last 5 years.
The statement of service must:
1. be on the employer's letterhead;
2. confirm the dates of employment;
3. describe the role in which you were employed, and whether it was full-time/part-time. If employed part-time or casual provide the total number
of hours and date of last shift.
4. be signed by a manager(e.g. Director of Nursing, Unit Manager or HR Manager).
Please note the statements of service provided cannot be accepted as they do not
confirm your mode of employment. Please contact
a new statement of service be issued that clearly confirms your mode (full time / part time
/ casual) of employment.
Expiry date of application
If you do not provide the above information necessary to complete your application,
AHPRA will be unable to progress your application for registration. This information is required by close of business 7 May 2014. If you do not provide the information by that time AHPRA will consider that you do not wish to pursue your application for registration
and this will be withdrawn, and to ensure AHPRA's compliance with State and Territory Public Records Act/s, all documents received from an applicant or prospective applicant, will be retained by the Agency.
