3.Record of immunisation. Evidence of immunisations in line with NSW Health requirements must be provided. Fill up information sheet 2, form 2 and form 3. Hi guys i sent my application last November. Im jut wondering why is it that ahpra didnt email me to provide the evidence for accreditation. Do you guys think were all required to send the new requirement?

Hi joejoe. The new model was introduced last February 10, 2014. It is effective for all applications incomplete at or received after October 7, 2013.

I think I should try going to CHED too. PACUCOA needs endorsement letter from the school. I already have my own letter and attached documents but the lady required me to have an endorsement. I asked her how much is the payment and she said its 1,000 php.

went to CHED NCR in UPD 2 weeks ago and they accepted my request. I just submitted the template from AHPRA and my credentials i.e TOR, diploma and RLE that were stampled as certified true copies by my school. they also asked for a P40 fee.

Where exactly in CHED? To whom should I go to? What do they call that form?

What I did for ahpra's Iqnm requirments is this.

For Iqnm 2, I just requested that my school send a certificate if accreditation (from Paascu) that the school was accredited, along with TOR and Ahpra's letter with their format.

For Iqnm 8, I requested also the school to send a course description and syllabus that I took Drugs and Solutions and Pharmacology duting my College

But for Iqnm 3, Im not sure yet, but I will pass the letter to PRC. Do u guys think BON is under Parc? And they can provide that letter??

Also, is it a requirment to pass a Referral letter from my previous manager's??

Hi joe. Just email your case officer that you heard about the recent changes and you've made arrangements regarding iqnm 2 and 3.

My school provided certified true copies of the certificate that CHED awarded for accreditation together with thr required docs.
