Hi rein, thanks for replying, yes, Ive confirmed that its really PACUCOA who accredits my school. Its just really frustrating bec they dont want to listen when its really going to save me time if they followed the instructions. Is it okay if you could send me a copy of the request letter of your officer? By the way, they are now charging 1000 pesos for that document so its important that they follow the instructions. In addition to that, I work in Cabanatuan city so I cant always go to manila in order to straighten out the documents that I need. Thanks so much again, my email is [email protected]. Azalie28, same thing happened to me. One, PACUCOA required me to have an endorsement letter from school. Second, the lady said that they cannot fill the template of AHPRA since they have their own. Lastly, she said that 1000 php is the fee.

yeah! she said the same thing! but its stated there in the instructions that it needs their letterhead on it together with the template, Have you tried getting the document with an endorsement letter from your school? Did they finally agree to print it out when you presented that letter? They were insisting that it should be my school who prints out the letter format that they are supposed to fill. Of course when my registrat read the directions she wouldn't do it and said that its not a form for the school. The staff at PACUCOA dont want to listen to me when I told them that They have a separate and form. Geez, its just following simple directions and they're giving us a hard time, on top of that 1000 is not cheap right.

Haha. I was decent enough not to argue with that woman because the staff are very accommodating. But yes, the woman who talked to me about the issue said that that school that I graduated from should fill up the template that I presented she even said the same thing she told you that the letterhead should be from the school. I told her that I already requested for the same certification from the school and that my school is on the process of accomplishing it. She insisted that they have their own certification.

Well, I never return back to PACUCOA. I decided to go to CHED instead. I requested for CAV at CHED-NCR with my own request letter, AHPRA letter and template, a photocopy of how my school accomplished the AHPRA form, certified true copies of rle, tor and diploma. I paid 40 php for that. Fill up their forms and was asked to make a call by next Tuesday, a week after my request.

yes, ched has released a certificate with the same template from ahpra also stating that my school and the study programme (makati med)

are both recgonized by ched.

my sister sent posted them through today.

For criteria 3, i went to paascu and ched office and gave the template and letter to them. For paascu, photocopy of tor and diploma are okay for ched, they need the certified true copies.

After completing this, are we going to send it to the address at the template or to where we submitted our application?

you'll have to send it to the address in the template. I asked this to my case officer, and I was told that all information requested must be sent to the NSW office for evaluation.

i lodged my application to ahpra- qld. Do you think the address in the template should be qld's address or should it be exactly what it is in the website?

just to clarify my earlier post. I had lodged my application in Victoria before the change of framework of assessment. I was also confused so I asked my case officer, this is exactly what she said..

"According to the correspondence issued to you yesterday all of the information requested must be sent to the NSW office for evaluation of your qualification."

So, I did not change the address of the letter.. All of the documents provided by my school and CHED were forwarded to NSW.
