my friend applied last week and they told her that she can be given a slot September or October if someone would back out. But if not, she can join the January intake.

Question on having documents notarized

I just had my documents certified by a notary public. I showed him a printed copy of AHPRA's instruction on certifying documents. BUT this is what he did.

For 1-page documents, he printed on the back of the document the following:

This is to certify that the [title of document] presented to me by [my name], whose signature appears hereunder, duly issued by [institution/facility who created the document], is a faithful reproduction of the original document.

In Witness Whereof, Affiant hereunto set his hand this [date today] at [town, province].

___[my signature]____

[my printed name]

Affiant

Subscribed and Sworn to before me, this [date today], municipality of [town, province].

Doc. No.: ____________

Page No.: ____________

Book No.: ____________

Series of 2012.

It was "sealed." And on the right side, there is a stamp which includes his name, and the title Notary Public.

I repeat, this was printed on the back of 1-page documents. And for documents with more than 1 page, he printed the above statement on the back of the last page. He initialed every page before that.

What's wrong with this picture? HELP! Suggestions would be appreciated.

Mine is like this... this is to certify that this document is a faithful reproduction of the original document as presented to me.

Then ________ City, then (Date)

Then Lawyer's name and other information...

I did not have to sign the document.

Hi @chelseaxyrene!

Thank you. Did the authorized officer print that annotation on the back of the document, too? And AHPRA accepted your notarized documents as valid, correct?

I'm thinking of returning to the Notary Public tomorrow to rectify this. I'm just wondering if it would be ok if the following annotation would be handwritten instead of printed or stamped on the front page (instead of the back page): "I have sighted the original document and certify this to be a true copy of the original"?

However, if your case is similar to mine (in that the annotation is on the back page) and those notarized documents were accepted by AHPRA, then I might not return to the notary public and instead lodge the application tomorrow.

my lawyer stamped at the front part of the document. there are those that were stamped at the back part because the annotation won't fit the front part and it was accepted by AHPRA VIC. I don't know if they will accept handwritten annotation. However, I have friends who lodged their application in AHPRA WA and all the annotations were done at the back part of the document. AHPRA WA accepted their documents and they now have their LOE.

Really? How did she apply? Just sent the docs via mail?

I don't think there's an intake in january

I'll ask her. Maybe she inquired via email. There's an intake on January. I just checked their website. January 7 2013 for Melbourne and January 28 2013 for perth.

That's good news! :)

Follow up: For documents with, for example, 3 pages, the annotation is located only on the third page. The notary public signed his initials on the first 2 pages. Is this what was done to your documents, as well?

Sorry if I ask too many questions. I paid thousandsss for the notarization and I hope it won't go to waste.

Thank you, @chelseaxyrene!
