Mine was stamped at the back. I was the one who actually requested it to be at the back so as not to cover anything (words in this case).
The notary public didn't have any stamp showing his telephone number so what i did was I sent the official receipt from the atty's office (it has tel #) and wrote some note why I included it. So there!
Just think on how to satisfy AHPRA's requirement.

No. Each page is notarized. I think it has to be that way. Not just the initials.

It was on the stamp.
Yes you have to go back because chelseaxyrene is correct and this is clear in AHPRA's website as well.
Request them to put anything that will satisfy AHPRA. Ex. TOR - put page 1 of 3, page 2 of 3 and so on.
@sendmethere: actually, I printed the instructions on certifying documents from the AHPRA website and I didn't see that specific example. Now it makes me wonder if I have the updated instructions. hello to all, I am a newbie here and just read almost all posts, I have started processing on my application, but I am worried since I don't have much hospital experience, does AHPRA really stick to hospital experience only? Because that was what I was told, that my chances for an approval is slim; I was a CI for 4 years, mostly on RLE exposure, but I am having a hard time securing a SOS or COE from my previous employer stating that I was assigned in the hospitals most of the time. Currently, I am working as a clinic nurse.
Please shed some light for me please.
