I'll start off with basic anatomy and assessment. If there's anything that needs to be really covered, just let me know and I'll come up with it. :)

Primary Functions of the Respiratory System:

1. Provides oxygen for tissues.

2. Removes CO2, and waste products.

3. Sense of smell.

4. Produces speech.

5. Maintains acid-base balance.

6. Maintains heat balance.

Some anatomy:

Nose: humidifies and filters inspired air.

Sinuses: open cavities in which air goes through, located behind oral and nasal cavities, provides resonance during speech.

Pharynx: behind oral and nasal cavities.

Larynx: located above trachea, and below pharynx, commonly known as the voice box, contains vocal cords.

Epiglottis: closes when swallowing occurs.

Trachea: located in front of esophagus, bifurcates into the right and left lungs via carina.

Alveoli: basic units of gas exchange, cells in these sacs secrete surfactant--a phospholipid protein that reduces the surface tension in the alveoli; without it the alveoli would collapse.

Lungs: lungs are located in the pleural cavity in the thorax. The diaphragm helps in the inspiration process.

In order to assess how well a patient is receiving oxygen into his/her system:

1. Assess color and VS.

2. Capillary refill of

3. Assess mucus membranes.

4. Assess for clubbed fingers. (If these are present, barely any O2 is getting to the patient.)

5. Use Pulse Ox. (normal value: 96%-100%--anything lower and O2 needs to be instituted because hypoxemia is occuring--about 2L.) This can be placed in a pt's nose, earlobe, or finger. A SaO2 less than 70% is life threatening.

6. Assess if accessory muscles are being used=labored breathing, and this could cause the pt more anxiety. Inc anxiety=less O2.

7. Auscultate lung sounds.

8. Assess, assess, assess! :D

Hello all,

I would first like to start by saying I'm a new grad. as of January 30th! I had my exit on the 30th, and my class will be having it's actual graduation ceremony on Feb. 28th :balloons: Woo hoo!! Now, having said that, I have been out there surfing the net right now for NCLEX review sites. Our program purchased 3 weeks of review via the NCSBN website, however I did NOT find this site helpful in the least. As a class we were required to complete 4 hours of study using the NCSBN site, and as a class we despised it :madface: The questions were often on disorders we never covered and meds. we'd never heard of, the entire review seemed more geared towards RN's than PN's.

So, now that I am on my own and able to seek out my own review, I am looking to fellow graduates for advice. I noticed a few of you said you have been using the Kaplan Q-Bank review, which I just finished sampling today. I really found it to be helpful. I am pretty certain that I will purchase the review, which I think consists of 1000 review question... is that correct? To those of you who have purchased it, have you been using it alone, or in conjunction with some other form of studying? I am finding it easier to use resources from the internet rather than sitting down with a book.

One other question, is anyone aware of any online review games? A friend told me about NCLEX PN Review Jeopardy, but I haven't tried it yet. Well, I look forward to meeting you all!! Good luck with your studies!!

......congrats to you on grad.!:w00t:I know getting that part over is huge! Now youre onto other things like playing the waiting game!! Anyway as far as me, yes I have purchased Kap Qbank only... no more Kaplan stuff, and I find it rather frustrating. I only score 50% and it has made me extremely uneasy on if I ought to reschedule my NCLEX... so instead I went back to my old way of studying which is a file I bought some time back last yr. and it goes over everything you need to know pulling out disorders, meds., lab values. etc etc. I love this file and have passed it on to others... so instead of doing an entire review.. this works exceptionally well pulling it all out and to the point. I have also done a variety of books... saunders/lippincotts/NCLEX-PN made incredilbly easy... etc etc. although I think nothing can prepare you for NCLEX! just review all you know lab values , meds and side effects, precautions, etc etc etc and go in there and take it! My class mates took his saturday and said it was OFF THE WALL... it made me more nervous... but Ive decided Im going through with the rest of my reviewing and going and taking it THI THURS. and just know in my head and heart I have studied for 4 weeks and see what Im about!!! If you want my file or anyone else... let me know I can pass it along.. good luck!!

Thank you for the welcome! :) I appreciate the links you provided, cardio is something I can always use more brushing up on :redpinkhe Simple concept, yet so complicated. I think your philosophy is a good one, just study, have some confidence and go in knowing that you studied. That's about all we can do, I guess. And it doesn't help either, that each one of us is different in our learning styles so there is no one way out there that is guaranteed to help us master the art of the NCLEX :banghead: I really wish you the best of luck on Thursday!! How's the waiting process where you are? Do you have the benefit of the 48 hours or are they making you suffer for weeks... weeks, how cruel is that? I would just like to apologize to all those who are forced to wait for weeks! I may ask for a copy of your file. I think I will go ahead and buy the Kaplan and mix in some other methods. Thanks again!

someone correct me if i am wrong...

my answer would be bronchial...becoz.. the chest walls of children are quite thinner as compared to an adult .. but it does depend on whether this is an infant/toddler/preschooler/schooler/adolescent..normal findings on auscultation include:

  • loud, high-pitched bronchial breath sounds over the trachea
  • medium pitched bronchovesicular sounds over the mainstream bronchi, between the scapulae, and below the clavicles
  • soft, breezy, low-pitched vesicular breath sounds over most of the peripheral lung fields.

Papillon , send me your email when you have a chance I can send you that file ... and unfortunately I have to wait 4 weeks after NCLEX... Im shocked of all states CA doesnt , but I honestly think its a LVN-RN thing... because in CA LVNs are kinda put on the back burner so to speak ... oh well, I have 4 weeks to do other things... I think just having the nCLEX done will be good..
