Long term goals for "Impaired Swallowing" due to CVA?

Published

This is my first post on this forum, and I hope I'm in the right place.

I'm doing a care plan on a pt who had a CVA, and one of my nursing diagnoses is "Impaired Swallowing". I have my short-term goal done (Pts throat will remain open/clear of saliva accumulation (timeline: end of shift), but am having a hard time w/coming up w/a long-term goal. This pt may not regain enough swallowing ability to eat/drink, and other than that I don't know what goals you'd have long-term. He has a PEG tube, if that helps. He is totally aphasic, although he no longer has a trach, and normally does not need suctioning for saliva accumulation (he had to be suctioned the other day b/c a tech had dressed him and left him sitting slouched down w/his chin on his chest in his WC. His O2 Sat dropped until he was suctioned and positioned in the WC properly.

Can someone get me going in the right direction?

Thank you!

I really can't help you. All you posted is assessment information. Goals are based upon the predicted results of the nursing interventions you order and you have provided none of your nursing interventions.

His swallowing is not the biggest problem now since he is NPO. His risk for aspiration 2o PEG tube feeding is. Aspiration pneumonia is a huge problem for these folks.

Go from there.
