RaDonda Vaught made at least 10 mistakes in fatal Vanderbilt medication error, prosecutors say

This article outlines details of the prosecution's case. I am curious as to how the defense plans to pin this on the hospital?

I still can't fathom why the Tennessee BON didn't see fit to require at least some reeducation of Ms Vaught!

Maybe a paralyzing agent doesn't need to be put in a suremed or pyxis or whatever. Maybe only anesthesia people need to have it.

It is in RSI kits in many EDs where I have worked over the years. Anesthesia isn't always available in some facilities to respond when we need an airway. But maybe part of the safety feature here is that it is in a kit specifically for RSI, so you really can't make the same kind of mistake as occurred here. At least, I would hope!

Wow spending a little extra on the kit seems like it would be well worth it. I can't believe the more I read on here. That hospitalwatchdog.org is a very good thing! Very informative. Did you see where the dr was doing breat implants with propofol in his office in Florida and not even anyone knew how to check a pulse even when he lost the patient on the table in his office it appears? Very sad.

I can't believe VUMC even hid it from the family who just found out from the media almost a year later. Unbelievable!!!

I can't believe there were 3 sentinel events in such a short time at the same facility. What are they doing?

The name 'Help All' nurse is so unprofessional sounding. Kind of like Jack of all trades...

What were the other 2? What does their Director of Nursing have to say about it?

Umm the date on this document by the signatures is the beginning of December.. To my knowledge no charges were filed until 2 months later. Is this sufficient for you TriciaJ?
