RaDonda Vaught made at least 10 mistakes in fatal Vanderbilt medication error, prosecutors say

This article outlines details of the prosecution's case. I am curious as to how the defense plans to pin this on the hospital?

I still can't fathom why the Tennessee BON didn't see fit to require at least some reeducation of Ms Vaught!

You feel free to go back and read if you want Wuzzie. One simple example is you said maybe my computer was messed up that the versed label was actually orange. I asked you simply if it was the lid or the label that was orange. You toatlly ignored that really simple question.

Also twisting my words again...I never said it was you saying that 'Ignoratio' thing. I never even implied it. What's the purpose in implying I did. How does that benefit the discussion? It's easy to see who said it. It's just not very nice but whatever I'm a big girl!

She should have never had paralyzers in her pocket in the first place, which is kind of the whole point and part of where it all went wrong...Also 2 years is still fairly new, but 2 years starting as an ICU nurse is much different than 2 years starting as a medsurg nurse. I started in PICU myself and was far more "advanced" than my med surg counter parts. So yes it is reasonable that she was in a role like this.

Normally I agree that these significant errors and sentinel events are caused by a multitude of factors and can be prevented/improved from a hospital-wide multidisciplinary type of approach. Here, many of those safety features that would normally be placed after an event like this were already in place.

If she practiced as a prudent nurse and then these errors still happened, I would 100% be on board with you that this is not just her mistake. But she did not practice as a prudent nurse, she negligently blew past any and every single safety feature that was in place to prevent such a thing from happening, and she outright admitted it was her fault as well. I just can't justify that. No matter what is changed and what is tweaked, she still would blow past the safety features like she did.

For what it is worth, every facility I've worked at in my career the "nurses" that are in these other departments like xray, CT, etc never give any of the floor-ordered medications. This is always the responsibility of the bedside/floor nurse. The nurses in those departments are responsible for the departmental-nursing tasks that I shouldn't be performing, such as injecting contrast dye and recovering them from the conscious sedation they are given in MRI, etc. That's just been my experience though. ETA: Some of them won't even turn of a syringe pump that is complete for you because that is the floor nurse's medication and thus the floor nurse's responsibility.

I did not say your computer was 'messed up". Sometimes the colors aren't accurate. Everybody knows this. Sorry I missed the question in the wall of text. I was focusing on another point that I thought was more important, but to answer your question both the label and the cap are orange.

You wrote this to me. Not to the person who used the term you found offensive but to me. How was I to know that you were speaking in general terms? Then you wrote this...
