Oct 27, 2014 Can you give us a little context?If I had to venture a guess, I would say that it's a log of all client contacts, including phone calls. If you talk to a client, be it over the phone or in person, that needs to be on the log.

This could mean the telephone contacts that are allowed to call about the patient's condition, or the contacts the patient allows for whom their condition may be discussed. Most facilities have a registry of all the in-house patients with their room telephone numbers. So this request could be interpreted many ways!!
