I'm hoping that someone out there can give me some advice. The facility for who I work filed a complaint to the health department. Has anyone ever had that happen? I have been nursing for 12 years and never had this happen? Please let me know thanks!!! I could use any information possible. Thanks!

Absolutely agree with the above and do NOT accept "but it will only make you upset" because that is bullying pure and simple. It is also a denial of your rights. Grit your teeth. Sit down now and wirte out the incident so that it is bare fact - just that nothing more no excuses no rationales just bare facts. Do not shift blame to others and especially do not shift blame onto yourself - just bare facts. Then look at it objectively and trace what led to what and why and again do not blame shift especially onto yourself.
Nurses are very good at sayng things like "I suppose I could have tried harder, done better, been more perfect." That is not part of the equation.
