Hello, new to allnurses, need to find a good worksheet

I have been making daily worksheets for pt. data for about 5years. They are hand made and I just haven't found a real good one. Does anyone have one they could send via e-mail?

Here is a sheet I made and used when I worked on a cardiac medical unit. I printed it on 8 1/2" x 14" paper front and back and kept it in my pocket at all times.
