I am just starting out my degree in medical transcription and I am doing a question about GP "codes".

Can someone please explain this to me...

para 1-2-1-3....

That's the order it gave me

Gravida 3, para 1-2-1-3

I also have this one:

Gravida 1, para 0-2-0-2

It is term, preterm, abortion (includes elective and miscarriage) and living

So the 0-2-0-2 would mean she had 2 preterm deliveries and 2 living children. They would have been pre term twins since she is G1

That's actually impossible. The first two numbers after the P (the 1 and 2, which stand for term and preterm) refer to PREGNANCIES, not children. If she's pregnant with twins, you don't count it as 2 pregnancies. That's what the last number is for (the 3, which is the "L" in GTPAL, which stands for living children.

According to the Para numbers, she's had 1 term pregnancy, 2 preterm pregnancies, 1 abortion (either miscarriage or elective abortion) and she has 3 living children. If those numbers are correct, then her Gravida would be 4, not 3.

If she had preterm twins, then she would be G3 P1113.

That one is also incorrect. You can never have Para numbers that exceed the gravida number. Assuming these were preterm twins, she would be G1 P 0102

It's something even a lot of OBs and midwives screw up.

Klone, yes! I forgot about that since transferring to the NICU. When I first transferred I tried to get others to document it the right way but stopped trying after nothing changed.

Yes, thank you for completing my incomplete thought. T and P in GTPAL refer to PREGNANCIES, that lead to delivery, that take place AFTER 20 weeks (before 20 weeks would be in the A/Abortions category).

So many medical professionals mess this up. I have to admit sometimes a woman's reproductive history can be confusing, which doesn't help.

I don't know if they make sense or not, but I didn't make them up. This is a critical thinking exercise in my textbook.
