I did an anecdote where i`m supposed to reenact exactly what I did when giving my IPID push med-- (I gave protonix iv if anyone is wondering).. and I "thought" that if I just wrote that i reconstituted it with NaCl, and administered at a rate of 2 mins .. did not contaminate.. and flushed line before and after that that would be the end of it. I get the anecdote back saying there needs to be more detail.. I`m trying to figure out what else was I supposed to do .. i could also write that i did the 6 med rights.. assessed IV site for infiltration, phlebitis, etc.. wiped the port.. flushed with 5 ml.. gave 40mg/1 ml over 2 minutes... then flushed again.. am i missing something crucial??

What you wrote sounds good to me. I'm not sure what else they want. Maybe your assessment of pt's condition warranting the med in the first place.

Write down that u took air bubbles out?? Advised patient to let u know if they feel any burning or pain at the site??

Do you think that they might want you to include everything you did from start to finish IN DETAIL beginning with that you checked the MAR, removed the med, verified it was the right med. How you prepared the med, drew up the med. How you verified the med and the patient. How you verified that the IV was patent.
I believe they do want.....I withdrew the med from the pxys machine. I verified that the med was Protonix 40mg. I then prepared the med by drawing up in a syringe....blah blah blah.........I identified the patient by the med rights. I assured that the IV was patent by aspirating back and visualizing a blood return. I then flushed the IV with X amt of saline I then proceeded to give the Protonix 40 mgs.........blah blah blah.......
I am sure That is the detail they are looking for..
