I figure since this is the largest nursing online board someone may have heard of her. Her name was Theresa Ames now it is Theresa Hardy. Try going to the web site for Dept. of Health if you know what state she's licensed in. I don't know if all states do it the same, but in mine you can look under licenses by typing in a person's name and their mailing address comes up along with their info.
