To start things off, the best and funniest order I have seen on a chart, was in the discharge instructions for a trauma patient. It read simply
Darwin Consult
and was signed by the resident. Well the attending did laugh, but it was not the highpoint of that residents day.

I've not seen it in Michigan. Is it given IV push? What's the generic name? Very interesting...
It may be a Europe/Australia/NZ thing; all of the websites I've seen that involve IV acetaminophen have all been from Europe, Australia or NZ, although there are some clinical trials for the drug coming up on clinicaltrials.gov.
-May have up to 5 cans of soda per day, each shift to document # of cans of MAR
-Pt. may have NO POPCORN on 3-11!!!!!!! (written like that, f/b, Pt. MAY HAVE popcorn on 7-3.)
-Pt. may have 2 double shots of whiskey prn for pleasure (Patient was a tube feeder, non verbal)
-Pt. may have ear pierced on July 12, 2007
-Nurse may slap patient across face if patient becomes non-responsive
-Pt. may have 240 cc's WARM TEA from Dunkin' Donuts flushed through GT on 7-3 shift for pleasure.
Then I had a doctor ask me in a serious tone "What is a LPN? Are you like an Aide?"
Good for you!!!We had this poor guy in the hospital who lived on the street. The doctor who was taking care of him thought he was faking his symptoms just to get to stay in the hospital and so he did not want the patient to get to comfortable there. The doctor went into the room and unplugged the TV and wrote an order that said "no TV". So I come to work 3-11 and this man's room is just outside my desk (I am the 3-11 charge nurse ). For the first couple of hours of the shift he kept yelling "I'm dying in here". I kept having to go in there to explain to him that he was not dying. After that, he started yelling "I'm not dying in here". I just could not take it any more so I went in there, plugged that TV in and gave him the remote. Never another peep out of him for the rest of the shift. Next time I saw the doctor I told him to never ever write an order like that again because we ain't doin it!

I have given IV paracetamol many times - as sairin8 says, it comes in a 100ml glass bottle and is run over half an hour (or maybe an hour). It is way more expensive than any other forms of the drug and so is used sparingly, when PR or oral dosing is inappropriate.
That said, when I first saw an IV order I thought the prescribing doctor was loopy! It has only been around, at least on the wards here, for a couple of years.
