Oh shoot, really? The patient population I was more interested in using the Neobar with was the over 3kg crowd - mainly the PPHN babies on paralytics because their mouths are always so gooey that their tape gets gross and loose. When I got the sample pack I thought there were six sizes, and that the biggest one was for kids like 4kg or so...

The largest size NeoBar, I think it's the peach one, says something like "for babies over 2200 grams" on it. We were using Neobars on all sizes of babies, but found that with the larger and more active babies, the Neobar would come off their cheeks more easily.

We used to prep our baby's skin with benzoin, but do not use anything with the Neobars, just make sure the skin is dry. I wonder if that made a diference.

We do not have a L and D at our hospital, and that may make a difference, too. All out babies are transported in, and their faces have already been "cleared" of birth matter. We will sometimes get transports of babies that are several months old and have never been home.

The largest size Neobar is blue, and they do fit bigger kids, even 8 pounders.

We rarely use tape any more.

We use Cavilon (the lollipop kind) to prep the skin, especially on the vernix covered preemies.

If you go the the neotech products page, the pediatric Neobar is peach-colored and should fit the larger babies, I would think...

Thanks! So far we haven't had a problem with the ones we have, but I'll check into the larger sizes just in case we need one. I don't know what Peds uses. Any kid that gets intubated on our Peds unit is transferred out.

I know I've seen some info about the weights for different sizes, but that info is not on the website. When I go back to work I'll look into that, too.

A while back, I requested samples of Neotech products via their website. Their rep was wonderful, emailing me information and sending me a bunch of stuff including a sample pack of Neobars - 2 of each size and the information about which color to use for which weights was right on the package. She also sent me a handful of EVERY neonatal product they made! Awesome customer service.

I've never used them on an actual baby, but when I was demonstrating it with a doll it was very easy to do with one person. I would think if the baby was flailing around it would be hard, of course, but if the baby was calm I believe it's supposed to be a one-person job.

We use something a bit like the clamp you were talking about but it's properly manufactured! Tried googling a picture but there isn't one.

Its a very soft piece of clear plastic/rubbery stuff about an inch and a half long (maybe 5cm) and 1cm width. In the centre is a hole just big enough for whichever size ETT you are using (the sizing coresponds) and it sort of comes up into a collar around your ETT. Around the collar is a hard plastic clamp with teeth on it. So when your tube is in, you take the end off it, slide the clamp on, tighten the hard plastic bit and this holds your tube in place (then put your end back on that attaches to the vent).

At each end of the clamp is a small hole. The baby has a hat with ties on it that tie into these holes and hold your tube in place.

No tape for prem skin and the plastic against their face is really soft and doesn't cause any trauma.

The company is EMS medical. The one I'm looking at says 'endotracheal tube clamp Ref SCP025 suitable for Portex 2.5mm I.D. (we use prtex ETTs).

Would the Neobar make retaping a one person job?

While some of our nurses are comfortable retaping with the neobar alone...I will never attempt it. Even if the person just stands there in case you need an extra "hand" it is not worth an extubation to me!

Our "norm" for retaping is one person is holding/securing the tube and the neobar while the other person removes the tape. Depending on how the person before you decided to tape the tube, this is either simple or a PITA. Before attaching the new tape we always double check the numbers/measurements and then apply the new tape...done.
