Whenever resources differ on answers, pick a reason to go with one answer over the other and move on. Most people go with the answer given by the resource that was published later.

The use of Demerol in pancreatitis & cholecystitis is quickly becoming out-dated. Due to its significant side effect profile many hospitals actually prohibit its use. It's true that it does not cause spasm of the sphincter of oddi like morphine (although one study showed the opposite), however it can cause a range of neurological sequelae and has a shorter half life resulting in more frequent dosing to maintain adequate analgesia. The argument against Demerol also relates to the type of patients who often present with pancreatitis - alcoholics. Alcoholics are more likely to have related neurological pathology and are therefore more susceptible to the dangerous neuro side effects of Demerol. The topic is worth a medline search to review the evidence - I'd recommend journal reviews above text books on this one.

AGREE. in saunders blue book it clearly states "administer meperidine hydrochloride as prescribed for pain because it causes less incidence of of smooth muscle spasm of the pancreatic ducts and phinctor of oddi (NOTE - although morphine sulfate or codeine sulfate MAY be prescribed, they are generally avoided because they can cause spasm of the sphincter of Oddi and increase pain."

New research stated Demerol doesn't lessen the spasm, thus, Morphine returned as a drug of choice because it works better. "Pass CCRN" book for 2014
