Hi everyone! I haven't seen a thread started for this yet so figured I should start one! I'm applying to CSULA's ABSN program for Summer 2016 and am about to submit my application in the next few days. Who else is applying? Figured we could all support each other and keep each other updated during this nerve-wracking process :)
The program director/my interviewer spent a good amount of time going over the health clearance form and emphasizing that I get started asap on making sure my immunizations/titers are done as some of them may take weeks or months e.g. Hep B series, 2-step PPD. I think that if you want to complete the health screening by the start of program and you don't have a lot of these things done, you would need to get started on it before decisions come out.
I'd be happy to take a photo of the form and send it to whomever didn't get it on interview day if you need it. Feel free to message me.

Did she give you a 3 page paper with "Required Documentation" on the first page, "Required Health Screening" on the second page and "Physical Exam" sheet on the last page?
Yep, 3 pages. I just completed the health req's today (pages 2&3). Now I need to work on page 1... No matter what the decision, I want to be ready.
For the Physical Exam, they just did a standard physical and filled out that last sheet? I have my physical next week and wondered if there was a more specific document for it.
I saw a PA and she just filled out the last page and gave it back to me during my physical.
As far as the titers go, she gave me the result of all the blood tests as a separate page. She wasn't sure if CSULA wanted her to fill it or if they do it based off my immunity results. Thankfully I have all my vaccines and immune to all so it's just a matter of checking off the boxes.

Definitely think it's when you get accepted, you will be provided with the info. Nobody mentioned to this to me during my interview either.
