    Yep, 3 pages. I just completed the health req's today (pages 2&3). Now I need to work on page 1... No matter what the decision, I want to be ready.

    Thanks @stomtom, I'm getting both done with my GP. I usually get the flu shot every year because it's kind of 'required' by the hospital where I volunteer, otherwise I'd have to wear a mask .
