Crushed Tabs and Liquid Meds?

I was wondering if nurses after crushing tablets for a g tube administration, do they ever mix crushed tablets with liquid Meds like liquid multivitamins, or prosource?

Or do we mix crushed Meds with water, administer, then after administer liquid Meds?

Best practice is to mix the crushed medication with water and administer one at a time. The rationale for this is because if something happens during administration- tube clogs, extension tubing is dislodged or the syringe pops off the port, there is no way to accurately determine how much or what medications were given if they are all mixed together.

Check for facilities policies for giving multiple meds at a time.

Generally most places have a policy against it.

However have I personally mixed meds in a single syringe to give via g-tube to save time. Yes

I am still in nursing school and my program taught me to mix the crushed tablets into water and administer them separately. "Them" as in mixing all the tablets into one cup with water.

I've had to do this about 3 times so far.

I dump all of it in a cup and then fill the cup with water and stir. Unless it's prevalite or an antacid. It mixes in the stomach anyway. Never had an issue.
