Can someone tell me if I am missing something with this mnemonic for the 20 min check, WIGSS CARE, silly I know lol.

Wash hands

Intro/Id pt

Glove up

Site check/gloves off/sanitize

Skin turgor

Check tubes/fluids infusing w/kardex

Assess pain/ comfort (if assigned)

Record info

Explain all procedures to pt

Don't forget I&Os. While doing your 20 minutes checks you want to explain to your patient that you will be measuring their intake and output. If they aren't NPO and they're alert, ask the patient to help you by noting any fluid intake they have while you aren't in the room, and ask them not to empty their urinal, let you do it, so you can get an accurate measurement for I&O on your final charting. Also IV intake if they have any piggybacks.

I want to say that Civita posted on the EPN in the last few weeks saying that your hands must be sanitized before checking the tubing/iv fluids. I'm going to clarify on the phone with them this week.

We have to gel before touching the tubing? Argh. There's so much to remember! Even with mnemonics I'm afraid I'm going to forget things!

I believe so. Civita had to check on it or something...I'm sure that one depends on the CE. You should see the list of questions I have to ask Excelsior. It's ridiculous.

So after checking skin turgor I have to sanitize again. So why can't I just check skin turgor on one hand and check iv site on the other while I still have on gloves :confused: I am getting SO confused, please let me know what you guys find out. TIA.

I will. I just want to take my own hand sanitizer at this point but it's not allowed.

I kind of figured that's what you meant, but everyone use different words. Thanks!

I just got off the phone with Excelsior and the person I spoke with said that we must gel our hands in between checking the site/tubing. Gloves must be off because you may have been contaminated with body fluids and it cannot be transferred to the tubing/fluid. CDC policy is to wash hands (gel) before/after glove use. All stuff I knew.........but that afternoon chat put my head into a spin!

Thanks for the Nemonic, so to clairfy, you left your gloves on for checking the tubing and passed? If you touched the site with the clean gloves, then pinched the skin, and went up the tubing to check for kinks & the med wouldn't you be contaminating the tubing and med bag? I think in Rob's ebook it says to take them off, but I have to double check. Please post if anyone knows for certain.
