Could charts be more disorganized?!

We were assigned our first patient today and our goal was to go through our patient's chart and find pertinent info. It took about 2 frickin' hours to find all of the info we needed because papers were misplaced in the chart, my patient had a Kardex in their chart as well as at the nurse's station and the info wasn't the same, the MD orders hadn't been updated in 2 days, etc. Granted, most of the documenting is computerized but still, you'd think that the paper charts could be a little more organized!! Sheesh!!:bugeyes:

This is why I spend about ten minutes per patient each night I work to organize the darn things. I hate a messy chart, but sometimes it seems like I'm the only one who cares about it!

I think what bothers me more is all of the illegible handwriting you find in most charts... :angryfire
