Case Load Home Care

Inquiring of caseloads for home care Hospice. Also LTC which for my organization includes ALF and SNF.

Our Home Care Caseload is 13 (although I am often over that #). LTC nurses are at 16.

I am in Washington State and have worked for two different hospice agencies. Each one tried to keep us at 12 cases or less, but, I have had as many as 20 for a very short time when we were understaffed. I hope that helps.

In Massachusetts, right now census is super low and I have 9, I've seen us go up to 22/23 when short staffed, I'd say average is 15.

agency goal is 12-14. usually it's 14-16, sometimes up to 18 when we get slammed with census out of nowhere.

My case load right now is 30... I'm also on call every other week for a whole week at a time... 5pm-8am then work regular work day then Friday 5pm-Monday 8am with literally no help whatsoever... lord help me!

When I worked home hospice / home palliative care my caseload was at least 18 and often more than 20.

The problem are the "additional" visits for pronouncements, crisis, and to cover somebody else.

This is unsustainable :( it's not safe for you or for the families you care for (or for the people who share the roads with you if you've been called out multiple nights in a row and are still working your regular days) praying that this is just a temporary situation and hat your agency is able to staff appropriately very soon. Stay safe out there, and Yes, Lord please help this nurse!!!

I worked at a hospice in the Southeast and my caseload averaged 12-14 patients but there were times where you had a busy week with admissions and end up with 16 or 17 patients which is very difficult to manage in hospice. The nurse who mentions her caseload is 30 patients and oncall for a week every other week, please find you another hospice company because the one you are with is DEFINITELY not the normal hospice company. They obviously don't care about the staff. I worked hospice for 8 years as a case manager, the most patients I ever had as a case manager was 17 and that was for a short period of time, my average oncall was 3 times a month with one of those days being a Saturday or sunday, not both. Please know that what they are asking you to do is not common to hospice.

Is this with an LVN? I'm at 29 with and LVN and going crazy!
