Let me begin by saying I love this website. I think the support I get from you guys is unmeasurable.
I am almost done with my nursing theory tests. I still have sociology left to clep. I am Currently studying ls3. So far, I have done alright up to this point. Some c's some b's and an A so far. i study the test guide and use Lisa's notes and sg101 and most of the recom. texts. I guess I am getting toward the end and I am getting very nervous.
I read about someone's cpne experiences and the care plans. Exactly when do you learn them( care plans) and the documentation? Did I miss something???
I try not to read ahead and scare myself, but too late. after reading somone"s cpne experience thread and I think I want to run my own head over. Kind of late to turn back now, but what to do?
I planned on taking both Excelsior and someone elses workshop when the time came....but I feel so behind. I guess my entire question is this... Are you prepared by the time you get to the CPNE??? Should I know about care plans by now??? what about the documentation?

I would recommend Sheri Taylor's online workshop. I went to the in-person and online version, but if I had to recommend one or the other I would go for the online one because of her amazing careplanning videos and webinars.

I posted this same thing about a month ago, and basically the fellow students advised to not get to ahead of myself. I am hoping that with the FCCA, we will get some concrete format. For now, I am using the careplans that Smeltzer Med Surg has at the end of their chapters. I too was horrified when I read the post of the failed CPNE for not documenting the the skin inside the nose and wondering what in the world I have got myself into but too late to turn back now. I have to take LS2, 3 and transitions and once I hit FCCA, I am going to 1st look at an online workshop and see what help that brings. Good luck.

The med/surg book is more the nursing process, but that is what I have been trying to do; In Smeltzer, look in the where the chapters are listed, and the nursing processes for the corresponding diseases are in blue. Doesn't our actual Fundamentals book have care plans also? I think so, I will have to look. I know, this is frustrating!
Excelsior doesn't "teach" you how to do careplans. You should have learned that in LPN school. But, that being said, most of us working in the real world have careplans that are already typed up and we just check the box next to all that applies.....so, we forget how to do them. When you get your CPNE study guide there will be a section on careplanning. But, as I've said before, I don't have any idea what other workshops are like, I just know that Sheri's completely prepared me for the charting I needed to do at my CPNE. You have enough things to worry about that weekend, and charting shouldn't be one of them. Here's an example, if on my Kardex, I was assigned respiratory management, (incentive spirometry or cough/deep breathe) my care plan would be Ineffective airway clearance r/t retained secretions. You just match your assignments to your careplan. It becomes very easy after doing it for awhile.

Actually, under my LPN program, designing careplans fell under the duty of the RN so this was not covered. This was all new to me. I graduated back in 1990 however, so things may have changed. We learned all about documentation in the chart, SOAP, etc....but we were not allowed to design a care plan.

I also didn't learn care planning as it was not included in my scope of practice. However it seems somewhat straightforward I just want to be feel comfortable with doing it in a timely fashion. Especially as I've noticed more than one failure related to care planning. I did learn charting, documentaion, Soap notes, and the like.

It has been 13 years since I finished LPN school and they drilled care plans into our head. I think that is one of the main clinical things they went over constantly and they had to be perfect.
