Well, I can't tell you what your instructor wants, but I can try to help you some.
First, think about fatigue. How often does someone actually pass out in the floor because they are tired? Not often. You usually pass out because of decreased blood flow to the brain.
I'm not sure where the cognition thing is coming from. Obviously if someone is passed out they aren't going to be cognitive of what's going on around them.

basically there is this patient who was found passed out on the floor. he works at a restaurant during the nights. and one of the employees saw him passed out unconscious on the floor before he was brought to the hospital. thats pretty much it about the case study scenario.
the nursing dx i thought of is Fatigue related to ........................... aeb by passing out on the floor.
my instructor said its not fatigue. right now we are studying about the head. and my case study has something to do with the head. i ask our instructor about it, and he said that its cognition. something to do with cognition.
any help of what it could be? a diagnosis about the head?
