Im at a loss on how to answer these two questions. Can anyone help with some ideas??!!

1. Discuss the value of best evidence as a driving force in delivery of nursing care at your facility.

2. What safeguards and decision-making support tools are embedded in patient care technologies and information systems that support safe practice at your facility?

I'm not sure what the instructor is after for question Number 1, I would ask for clarification including an example.

For Number 2, I can think of a few off the top of my head… Think about Electronic Medical Records (EMR's). When a patient is scanned and a medication is scanned, a safeguard I can think of is a hard stops for creatinine or BUN results that are contraindications for certain medications. Another is a diabetic patient who had a CT with IV contrast who is scheduled to receive metformin within 48 hours. This would be a a contraindication and the EMR should stop the nurse from the administration of the metformin. An example of a decision-making support tool at my facility is a database called UpToDate which can be accessed directly through the EMAR for EBP articles.

Brainstorm and think of ways facilities in which you have worked or done clinical have fulfilled these requirements.

While it's nice when others try to "help" by posting their responses, this is your homework it's better for you to post your thoughts & findings and we will be happy to guide you to formulate an articulate response to your homework.

If you are blocked, post why these questions are stumping you and we will try and point you in the right direction.

1.) Research evidenced based practice... that answer is easy.

2.) This is referring to the implementation of the electronic medical record. For instance, when a temperature of 100.4 is recorded, and the WBC is greater than 10K.. it triggers an automatic stat page to the rapid response team. They come to the bedside and evaluate for sepsis.

This application for pharmacy notification and intervention is huge on it's own. A doc orders a drug that has been documented as an allergy. Pharmacy will not dispense!

OP, what is blocking you? What do you think these questions are trying to get at? Can you restate them using other language to make them clearer? That's often a good place to start, to help you avoid the facile, low-level answer.

As an aside, this question, and the fact that seems difficult, should be instructive to those who think getting the BSN credential is a simple matter of a couple of leadership and theory courses. Knowing the answers to these will lead the OP to a higher level of understanding of nursing practice than she currently possesses.
