Best Patient Care Plan book!

Please any one recommend the best patient care plan book. I am struggling with care plan a lot. Any advise on how to do a good care plan will be appreciated.

I love Doenges, Moorhouse, and Murr's NCP Guidelines for Individualizing Client Care Across the Life Span 7th ed.That CD is priceless to me...I should thank it at graduation.:wink2: Rationales are included too...

the best thing you can do is to learn how to use the nursing process to help you in writing care plans.

I just failed my 2rd (missed 1pt). Devastated; and can not fail anymore! If you can give me any info to better mysefl, I am great full. My weakness links are:

-PCP

-Clinical, especially charting, time management ( I still nervous about reading and understanding the "complete clinical picture" of my pt). I think I can fix this. but PCP is my biggest worrisome.

Daytonites and everyone: please tell me tricks to remmember a lot info, especially the S&S of disease when you first study about it. I need to graduate this program to get on with my life.

you need to know pathophysiology to give you the related factors (etiologies/causes) for your nursing diagnoses. you also need to review the s/s of diseases/conditions to double check that you didn't miss noticing any of them in the patient. you need to know what the complications of diseases/conditions and medical treatments are because you want to assess for s/s of them and/or care plan for them. and all of the above is
step 1 assessment
activities of the nursing process.

write it down. everything is related to something else. use the
critical thinking flow sheet for nursing students.doc
to help you study and learn medical diseases and conditions and how they are treated.

the flow sheet Daytonite ahs posted is great- concept maps reallly helped me fil in the big picture-- there is an online evolve.elseiveier site to create them that is great- it will all come together-get on board with Daytonite's flow sheet & a good concept map- your textbook and Mosby's nursing dx book have alot of ideas too- you can use trie and true- you don't have to make them up- boy I jumped for joy when I found that out! hth!
