thanks for fast respond! i understand what you said. but let say that i might not see the pt on the second day, i still evaluate all of my nis. what i try to understand is that i have to come up with a nx and eos (in my head) that is related to the pt's clinical picture. i have to do know all the nis for each eos so that i can care for the pt and have the evaluations and write them up later at night. that means i need to know a lot nis. that is challenging to me since i will be doing many nis in the care. some may be applicable to careplan and some not. and i am student, i might not remember all the nis. my instructor said that i can still have evaluation for careplan for one care since i could have done many things for the pt. please be patient with me and explain more! thanks sincerely,

Hi everyone, I am about to start the clinical, 2 semester. My instructor ask for a careplan and evaluation, even if I might not see my pt on the second day of clinical. She said that even with one day care, I still have done many things (NIs) to that pt My questions are? -To do a good care plan, should I come up one Nx and know all the related NIs (all in your head!) to care for the pt on the 1st clinical so that I can have evaluations for each NI ( let say that the EOs expected in 3 hr). Then I come home and write up a careplan. If I see the pt for 2nd day, I will evaluate more on the progress of the pt. Is that how you did the the careplan? -Do you have to know every NIs for every EOs on the clinical hours of first day? -In 4 hrs clinical, many things going ( give med, care for pt....) how can I have all informations for my careplan ( Nx, NIs and evalulation of NIs) in my head? It's like doing careplan while caring for the pt! Please help me with these! Sorry if I confused you. I am already confused and stressed about the careplan and clinical! Thanks:o

Sorry, I do not have a specific pt's case now. I am just wondering how to write the eval for a long term EO since my instructor ask us to come up with 2 EOs. One is for short term ( I think 2 days time frame) and one for long term. Any teaching from you would be appreciated! Thanks

Thank you kindly for your fast response! I hope to understand what you said. So as I wrote in the thread, I just evaluate to pt's responds to the NI(s) after second day of clinical and elaborate (according to the norm) more in the future of what pt's prognosis would likely be (assuming that the NI(s) is good)! Or should I not to elaborate? I asked this ? since my instructor asked us to elaborate the future careplan for the pt. Thanks:wink2:

Hi Daytonite and others, please help me untangle this, I have 2 days at the clinical/week. I understand that EO(s) have to be measureable and specific in time frame! My questions are ( I hope you understand my ?): -If it is an easy EO and in "2 days time frame", I might have the evaluation for care plan at the end of the second day in clinical ( let say for the same pt). -If the EO that needs more time, 3 days or more, what informations should I write for the evaluation? What should I ask the pt? Should I just write what the prognosis of the pt would be ( according tho the norms) without any response from the pt? Please tell me what I should do?? I think the careplan is more complex than just changing the tire as you sampled, :saint:Daytonite! I need to improve my careplan. Thanks for wonderful help!!:)

Thanks a lot everyone, especially Daytonite. I have been reading a lot your post. You are great helper. Thanks sincerely. I just failed my 2rd (missed 1pt). Devastated; and can not fail anymore! If you can give me any info to better mysefl, I am great full. My weakness links are: -PCP -Clinical, especially charting, time management ( I still nervous about reading and understanding the "complete clinical picture" of my pt). I think I can fix this. but PCP is my biggest worrisome. Daytonites and everyone: please tell me tricks to remmember a lot info, especially the S&S of disease when you first study about it. I need to graduate this program to get on with my life. Thanks again.

Please any one recommend the best patient care plan book. I am struggling with care plan a lot. Any advise on how to do a good care plan will be appreciated. Sincerely:confused:
